Respite Camp Application 2008-2009
Note: All forms require Adobe Acrobat Reader version 8.0 or later (Version 9.0 or later is recommended). If you don't have it, you can download Acrobat Reader for free. Please note that some Macintosh machines do not use Acrobat Reader as their default PDF viewer.
Print, complete and mail in the application with registration fee to save your place at camp. Campers are accepted on a first-come, first-served basis.

Step 2: Complete and bring on the first day of camp.
Health History and Physical Examination Form (not necessary for Fox Valley Day Camp Program)
Medication Form - Respite Camp or Fox Valley Day Program
Release of Camper Form - Weekend, Summer or Fox Valley Day Program
